Абстракт

Abstract Orthognathic surgery is a combination of surgical and orthodontic treatment, to correct abnormal skel- etal relationship that cannot be easily treated with orthodontic treatment. The main treatment objec- tives for serious skeletal malocclusions are aesthetic, functional and psychological rehabilitation. The diagnosis and treatment plan of orthognathic cases require a systematic team approach. An ideal team should consist of an orthodontist, a maxillofacial surgeon, a clinical psychologist, a prosthesiolo- gist and a specialized dental technician. The orthodontist can move teeth and alveolus but this has no substantial impact on adult basal jaw bone. The orthodontist’s main task is teeth alignment. The oral and maxillofacial surgeon is responsible for the surgical correction of jaws and their associated structures. Orthognathic surgery involves risks and this is why we need to proceed with it only after careful planning. It is particularly important to understand the patient’s concerns as it establishes trust and communication and helps to achieve high quality results. The management protocol for dentofacial abnormalities, treated through the cooperation of orthodontics and orthognathic surgions involves tak- ing detailed medical and dental history of the patient, clinical examination, cast analysis, photographs, radiograph analysis (leading to diagnosis and treatment plan), presurgical orthodontics, actual surgery and postsurgical orthodontics. In this report, we present a 16–year-old female with jaw deformation following odontogenic inflamma- tion, who undertook surgical revision and removed right condyle when she was a child. The team had chosen conventional approach to the patient. Presurgical orthodontic management includes leveling and aligning the arches, relieving crowding, creating proper inclination of incisors, coordinating upper and lower arches. The operation and recovery were uneventful.
