Assessment of mongolian health insurance Coverage among vulnerable groups: Migrants and poor

Абстракт

The Health Insurance (HI) system was developed in 1990. The Mongolian Health Insurance Law was adopted in 1993. The HI Law has been modernized and adapted five times due to social reforms, market stabilization efforts, population income, and changes in the ability to purchase. According to the micro study of 2000 on internal migration in Mongolia, around 30% of all migrants moved to Ulaanbaatar, Darkhan-Uul and Orkhon aimags. The aim of this survey was to explore the association between the knowledge and information about the Mongolian health insurance system and utilization of health care services among vulnerable groups. Poverty in rural areas stood at 43 percent, while in urban areas it was 30 percent. The percentage under poverty line in Ulaanbaatar city (Bayarnzyrkh district 30,5 percent, Songino-Khairkhan district 38,2 percent) was higher than in the Orkhon and Darkhan-Uul aimags. Finding employment was difficult because of the lack of education and lack of professional skills. Most participants visited primary healthcare centers, especially Family Group Practices (47.0-57.3 percent) and aimag district hospitals. Among the migrants who live in Ulaanbaatar city 26.4-30.0% visited tertiary level hospitals and 17.2-22.9% visited private hospitals. Of all participants who live in other selected regions 47.5-51.4% visited primary health care centers. In this survey, 83% of migrants who live in Orkhon aimag, 82.9 % of migrants in Darkhan-Uul aimag, 77.3 % of migrants in Songino-Khairkhan district, 78.1 % of migrants in Bayanzyrkh district were uninsured.
