Surgical treatment of urinary incontinence in the Mongolian women

Абстракт

Introduction: Urinary incontinence and pelvic relaxation were revealed consistently, when diagnosis of women treated due to the healed perineal tear. Stress urinary incontinence was especially studied by the study. Ignoring above mentioned pathologies leads to the harm for women’s health. This work was done at Gynecological Surgery Department, Obstetrics and Gynaecological Clinic, State Maternal and Children Health Research Center (MCHRC) of Mongolia.

Materials and Methods: The purpose of the study was to determine the prevalence of urinary incontinence among women who live in Ulaanbaatar. The study involved 2384 participants, who were chosen as representatives of 234117 women. They were chosen randomly, based on estimation with statistical significance of 95%, capacity of study of 80%, RR -1.5 and OR-1.59. They were aged 20-80, of different profession and occupation and involved in momentum study. A specially   designed questionnaire was distributed to all women and collected back after filled out by participants. 511 patients with ages of 21-72 were involved in the clinical study and had undergone surgical treatment. Computer software Epi info 2000 and SPSS-11.5 were used for data calculation.

Results and Discussion: Combined anal incontinence was diagnosed in 11.7% (118 of 511) of patients with stress urinary incontinence and pelvic relaxation. In the study of urinary incontinence, its prevalence among Mongolian women was 32.2%. One pathology revealed by epidemiological study was anal incontinence and it was in 98 (12.8%) women. First degree stress urinary incontinence was 31.1% (159), second degree – 57.3% (293) and third degree – 11.5% (59) and 80.6% was in women with ages 30-49. Mean age of patients was 41.2±7.6.

Conclusions: When three different disorders of lower urine tract, lower genital tract and anus were diagnosed in one patient, triple surgery method which can be performed in frame of primary surgery was applied. Safety for patients and high effectiveness of this method were related to the amalgamated treatment on the basis of broad understanding “pelvic relaxation”. It is possible to introduce diagnostic and treatment methods of urinary incontinence which is applied in the study into the gynaecological practice of our country.
