Абстракт

Background: The epidemiology of acute and chronic pyelonephritis among children has changed with time. The purpose of the study was to investigate the current causes and clinical characteristics of pyelonephritis among Mongolian children.

Materials and methods: On 84 children who were admitted to National Centre for Maternal and Child Health under the diagnosis of pyelonephritis from 2004 to 2005, we retrospectively reviewed medical records. Moreover, we prospectively examined clinical symptoms, causes and causative mi- croorganisms on 199 patients who admitted to the centre under the diagnosis of acute and chronic pyelonephritis from 2006 to 2010. The urine samples were collected and cultured according to the standard guideline.

Results: Totally, 283 children with pyelonephritis were enrolled in the study. The male-to-female ratio was about 1:3. The peak ages of female patients were 2-3 years age group (31.8%) and over 12 years (31.8%). The chronic type (61.5 %) was more prevalent than acute pyelonephritis (38.5%). The main predposing factors for inducing children’s pyelonephritis are hydronephrosis (63.64%), vesico- uretheral reflux(11.69%) and hyphoplasia (10.39%). Out of the 283 cases, 172 cases (60.77%) were culture positives. Among the culture positive cases E.coli (59.88%), Enterobacter (26.74%), Proteus (6.40%), Pseudomonas aerugenosa (4.65%), Staphylococcus aureus (1.74%) and Klebsiella pneumo- niae (0.58%) were more prevelant agents.

Conclusion: Further studies are needed to determine why certain pediatric populations are at increased risk for pyelonepritis. The study findings support that it is necessary to take a careful choice of antibiotics for pyelonephritis among children.
