Абстракт

Background: Velopharyngeal gap is a dynamic structure; there is no single normal mechanism of the velopharyngeal closure.  Nasopharyngoscopy is an important instrumental tool for assessing velopharyngeal function. The usage of Nasopharyngoscopy in speech pathology practice for children with clefts in Mongolia started from June, 2012.

Objective: The purpose of this study is to analyze velar and pharyngeal wall-movement and velo-pharyngeal gap during nasopharyngoscopy on children with clefts, who participated in the first clinical trial.

Methods: Nasopharyngoscopic recordings of 22 (7 male, 15 female) children with and without operated cleft lip and palate were analyzed. The subjects’ age ranged from 4 to 16 years, with a mean age of 13. Ratings of movement of velum and pharyngeal walls, velopharyngeal gap closure, size and existence were analyzed 3 speech language pathologists. Video recording was obtained during repetition of different types of stimuli/syllables/single words containing high-pressure consonants, low- pressure consonants, nasal consonants, plosive, affricative and fricative sounds. The flexible fiberoptic nasopharyngoscopy of 3.4 mm in diameter was used for the evaluation. For the data recordings, we used Nationwide Children’s Hospital Nasopharyngoscopy rating form (USA) sample.

Results: Out of total 22 patients with repaired clefts 13 had cleft lip palate (CLP), 5 had bilateral cleft lip palate (BCLP), 3 had right cleft lip palate (RCLP), 5 had left clip palate (LCLP), 7 had cleft lip only (CPO), 2 had an unoperated submucosal cleft palate (USCP). During speech tests 15 children had consistent velopharyngeal gap, where 7 was inconsistent. With regard to size of VP gap 4 subjects were observed with slight/bubbling, 9- with small, 2-with medium, 5-with large and 2 were variable. Ratio of closure: 7 had coronal, 1-bowtie, 2-sagital, 12-circular patterns. Thirteen children showed symmetrical lateral pharyngeal wall movement and 9 asymmertical. None of 22 children with clefts had pharyngeal flap operation. 

Conclusion: Present study provides preliminary data are that is first in speech therapy practices in Mongolia. There is a need in clinical training of speech language pathologists as well as future co- operation in this field.
