An international prospective study was carried out to examine the frequency and risk factors forK. pneumoniae producing ESBLs.1 An analysisof 455 cases of bacteraemia in 12 hospitals in 7countries was carried out. The overall frequencieswere: 18.7% of K. pneumoniae were ESBLproducingand 30.8% of these were associated withnosocomial bacteraemia. The frequency by countrywas: South Africa - 3 1 % , Taiwan - 19%, Australia- 17%, Argentina - 14%, United States - 10%,Belgium - 5% and Turkey -4%. The predominantidentifiable risk was exposure to antibioticscontaining the oxyimino group. In Singapore, oneof the risk factors is also exposure to the use ofbeta-lactams for ESBL-producing K.pneumoniae.2 A case report is described and themanagement is briefly discussed.
