Абстракт

PURPOSE

            Define the causes and some risk factors of neonatal seizure in the local context

OBJECTIVES

  1. Determine causes of neonatal seizure
  2. Compare causes of neonatal seizure by gestational age and first manifectation of seizure
  3. Define association between neonatal seizure and some complications of pregnancy, delivery and postpartum period.

MATERIAL AND METHODS

A total of 391 eligible preterm and term neonates who attended the Department of Neonatal Pathology of the Children Hospital of National Centre of Maternal and Child Health and Gurvan Gal hospital during the study period were enrolled.  The data are collected trough questionnaire and laboratory investigation and EEG, brain ultrasound results.

Data were entered into SPSS program and descriptive and bivariate analyses were applied to examine the relationship between dependent and independent variables. 

RESULTS

A total of 391 neonates were subjected to the study and 142 (36.3%) were diagnosed as having seizures, reminding 249 (63.7%) considered as having without seizures. Hypoxic ischemic encephalopathy (86 %), brain hemorrhages (6.3%) were the etiology more frequently associated to seizures.  When considering the etiology by the age of neonate,   hypoxic ischemic encephalopathy (88.4-71.5%) was lead for both groups of full term neonates while brain hemorrhage, neonatal infection and brain malformation occurred more frequently among preterm neonates.  For 57 (40%) of neonates seizures were occurred within first week of life.  Development of first manifestation of seizure was differed by its etiology.  Seizure was developed at first 2 day of life for neonates who had brain hemorrhage while neonates with nuclear jaundice and hypoxic ischemic encephalopathy it occurred at 5-7 days of life.  Considering risk factor for seizure by course of pregnancy, delivery and postpartum periods, a significant association between neonatal seizure and threatened miscarriage, fetal asphyxia and preeclampsia ( P= 0.02, p=0.001) was observed. 

CONCLUSION

  1. In this study hypoxic ischemic encephalopathy (86.0%) was the main cause of the neonatal seizures. 
  2. A significant association between neonatal seizure and threatened miscarriage, fetal asphyxia and preeclampsia (P= 0.02, p=0.001) was observed. 
  3. First manifestation of neonatal seizure mostly (60.1%) developed at early postnatal period particularly at first 2 day of life of the neonates.
