Background: Ineffective or unsatisfactory treatment of pain leads to suffering from pain, delaying of recovery, converting to chronic pain and negatively affected on quality of life. The main method and approach of main relief after pediatric day surgery is intended for accurate measurement and management of postoperative pain to its decreased minimal level and subsequently creation of pain free peri-operative environment. We assessed the multimodal pain relief approaches in pediatric day surgery practices.
Materials and methods: In this case-control study, the multimodal pain relief (preemptive anesthesia combined with regional blockades) group consisted of 121 pediatric day surgery patients, whereas the traditional pain relief or control group comprised from 120 children. Pediatric pain intensity was identified according to age using numerical, visual, facial behavioral and CHEOPS pain scales. The clinical parameters, clinical vital signs as well as laboratory indicators were recorded every hour in the first 6 hours after surgery and every 2 hours in the subsequent 24-48 hours.
Results: The average postoperative pain intensity was 7.15±1.08 in the control group, while it was 0.21±0.61 in the multimodal pain relief group (p<0.0001). The mean durations of recovery room stay were 30.1±18.1 minutes in the control and 59.5±37.2 in study groups (p<0.0001). The mean duration of hospital stays of the control and study groups were 9.4±5.7 and 1.4±0.1, respectively. The average pulls rate was 111±3.9 per minute in the control group whereas it was 99±5.3 in the study group. The respiratory rates in the control and study groups were 24.0±1.0 and 22.7±1.0 per minute, respectively.
Conclusions: The multimodal pain management was effective for pediatric day surgical practices.
